Prescription Drug Insurance - CHANGES EFFECTIVE 01/01/2017

Prescription Drug coverage is for Active and Non-Medicare Retirees and their eligible dependents. Effective 01/01/2017 your Pharmacy Benefit Manager will be changing to CVS Caremark. New identification cards will be mailed to your home address mid-December. DO NOT THROW AWAY this very important mail from CVS Caremark. CVS Caremark can be reached toll-free at 1-888-202-1654.
On 01/01/2017 your Anthem card will only be used for Medical Insurance NOT for prescription drug benefits. Due to this change, Anthem will be mailing out new ID cards that exclude Prescription Drug benefits during the month of January, 2017. Be sure to show the pharmacy your new CVS Caremark identification card on or after January 1, 2017 to access your prescription coverage.
